  Acceptable Forms of ID

All acceptable forms of ID documents must be current, or if expired, issued within the last five years. Presented ID must also consist of a photo ID.

All acceptable forms of identification (ID):

• CA Driver's License or CA ID Card
• US Passport or Passport Card
• Other state's driver's license or ID Card
• Canadian driver's license
• Foreign Passport (stamped by USCIS)
• Military ID
• CA Inmate ID (if inmate is in custody) - Does not expire
• Credible Identifying Witnesses

Important Note: Do NOT submit any identification information to our office via e-mail. We cannot accept materials via email. You must bring your documentation to the Passport Hub Center in person.

Important: (No Cash) Only payment with credit/debit cards; or check/money order. Payment should be made out to: CSUDH Foundation.

The Cost for Notary service regulated by in California is $15/per document/per notarized signature.

Example: If two people (i.e., husband and wife) are expected to sign and notarize one (1) single document, then the cost would be $30.00.

What is a notary public?
A notary public is a responsible person over the age of 18 appointed by state government to witness the signing of important documents and administer oaths.

What does a notary public notarize?
A notary public notarizes any document in which an originator needs to ensure the integrity of the signer. A notary public verifies the identity of the signer, and that they are signing knowingly and willingly. This process helps deter the fraudulent execution of documents.

How does a notary verify the identity of the signer?
A notary will ask a signer for a document such as a driver’s license or government-issued identification card that includes a photograph, signature and some information describing the person.

Can a Notary Public prepare or assist in the preparation of any documents?
No, a notary public can only witness the signing of the documents, not assist or prepare any documents. To assist or prepare documents would be an unlawful practice of law. This keeps the notary’s position as an impartial witness to the signing.

May a Notary give legal advice or draft legal documents?
A Notary public is forbidden from preparing legal documents for others or acting as a legal advisor unless he or she is also an attorney. Violators can be fined or jailed for the unauthorized practice of law.

Can I be refused a notarization?
If the Notary is uncertain of a signer's identity, willingness or general competence, or has a good reason to suspect fraud, they may refuse to perform a notarization; this may be the case if he or she cannot be certain of a prospective signer’s identity, willingness, or understanding of what is happening at that moment. In addition, a notary may not notarize a document in which he or she has a financial interest.

Notaries should not refuse to serve anyone because of race, religion, nationality, lifestyle, or because the person is not a client or customer. Discrimination on any basis is not a suitable policy for a public official.

Can a notary certify a copy of a document?
State laws may vary, but in general, no. For vital records documents such as birth certificates and marriage certificates, the requestor should visit the local agency that holds these documents, such as a local county clerk. There are some limited instances when a notary may certify a copy, but it is best to check with the notary of your state.

What about immigration paperwork... can a notary public prepare or notarize immigration papers?
A notary may notarized papers those that require notarization such as the Affidavit of Support, but they may not prepare or file another person’s immigration papers unless he or she is an attorney or a US DOJ accredited representative.

The Passport Center at CSUDH is conveniently located in the Extended Education Complex on the northwest section of the campus. The Passport Center is located inside the College of Extended and International Education. 30-minute visitor parking is available in the front/north entrance of the EE complex.
